OUR COMMUNITY LIVING SERVICES 

Hope Haven, Inc provides a variety of community living services and supports in Iowa and Minnesota including but not limited to: home-based habilitation homes, waiver homes, residential-based supervised community living for children, supported community living drop-in services for children and adults. In Minnesota, this includes supported living services, in home support, and foster care and respite for children and adults.

Iowa Services

Hourly Supported Community Living

Hourly Supported Community Living provides flexible, individualized support for children and adults living in their own home or family home in Northwest Iowa. Staff support each person in building everyday skills, working toward personal goals, and increasing independence based on their individual needs and service plan.

Waiver & Home - Based Habilitation

Home-Based Habilitation services are provided in homes throughout Northwest Iowa. Individuals help determine how they want to manage their home and daily routines, while staff provide individualized support and instruction based on each person’s goals and service plan

Host Homes

A Host Home provider welcomes an individual with a disability into their home and provides individualized guidance, support, and care. Hope Haven works with providers to support each person’s goals, independence, daily routines, and connection to their community.

Respite

Respite provides temporary support for children and adults, giving primary caregivers time away from their caregiving responsibilities. Services may be provided in the individual’s home or in the home of a respite provider.

Individualized Home Supports

Individualized Home Supports are provided to a person and their family in the home and/or community, helping the person remain in or return to their home while building skills and strengthening natural supports.

Community Residential Services

Community Residential Services are provided in Southwest Minnesota in shared homes, typically with three to four people. Individuals help determine how they want to manage their home and daily routines, while staff provide support and instruction based on each person’s individualized Coordinated Service and Support Plan.

Foster Care Services/ Supporting Living Services

These Minnesota services support individuals with disabilities and mental health needs in participating in community life with the greatest possible level of personal autonomy. Services are person-centered and designed to support independence, choice, and community participation.

Respite

Respite provides temporary support for children and adults, giving primary caregivers time away from their caregiving responsibilities. Services may be provided in the individual’s home or in the home of a respite provider.

Eligibility requirements and funding options vary by service and by state. Hope Haven’s Admissions team can help individuals, families, guardians, and support teams understand available services, eligibility requirements, and possible funding options.

In-Home Family Support 

CHILDREN & ADULTS · IOWA & MINNESOTA

These services are provided to a person and their family in the family's home and/or in the community, to enable the person to remain in or return to the home. This includes training of the person and their family members to increase their capabilities to care for and maintain the person in the home.

Residential-Based Supervised

Community Living (RB-SCL) 

CHILDREN AGES 8–19 

We serve children ages 8–19 who have intellectual disabilities and/or mental illness, emotional, and behavioral disorders, using a multidisciplinary approach to meet psychological, physical, emotional, and social needs in a safe, structured, nurturing environment. We focus on skill-building in living, social, interpersonal, recreational, and community integration — connecting each child to education, health, and mental health services, with the goal of improving overall functioning, reuniting the child with their home when possible, and building independence. 

Host Home Provider 

NW & GREAT LAKES REGION, IOWA 

A host home provider shares their home and opens their heart to a person with an intellectual disability, providing guidance, support, and care — working on goals like shopping, budgeting, coping skills, cooking, personal hygiene, and community integration. Providers are independent contractors, with Hope Haven offering ongoing support, guidance, and training. We are currently seeking Host Home providers in the NW and Great Lakes Region in Iowa. 

Respite

Funding options include several of the HCBS waivers. Respite gives a person's primary caretaker a break from their responsibilities, provided in the home of the person served or the respite provider. Supported Community

Living (SCL)  

ADULTS & CHILDREN · NW IOWA 

Provided in Southwest Minnesota in homes that usually have 3–4 people living together. The people served determine how they want to run their homes, and each person creates a Coordinated Service and Support Plan; staff support these decisions and give needed instruction to help them achieve their goals. 

Waiver & Home-Based Habilitation Homes

(3–5 persons)  

NW IOWA 

Funding options include the ID waiver and Habilitation Services, in homes located throughout Northwest Iowa. The persons served determine how they want to run their homes; staff support these decisions based on each individual's plan and provide instruction to help each person learn the skills to achieve the goals in their individual service plan. 

Smart Living

INNOVATIVE SERVICE MODEL

Smart Living combines advanced technology with remote support to promote safety, independence, and personalized care. From our newly renovated Remote Support Center in Spirit Lake, trained professionals provide real-time assistance to individuals in their homes — enhancing freedom while ensuring peace of mind.

 

Homes are equipped with customized technology such as motion sensors, wearable devices, interactive video tools, and other assistive features designed to meet individual needs — empowering those we serve while helping us reach more people with thoughtful, flexible support

Hope Haven began in 1964 as a school for children with disabilities in Rock Valley, Iowa. Today, that same vision continues through local disability services across Iowa and Minnesota and our international wheelchair ministry around the world.

He smiled so wide, his whole

face changed.

James is 9 years old and lives in rural Kenya. He was born with a spinal condition that left him unable to walk. For years, his mother carried him everywhere. Last March, a Hope Haven International team fitted James with a KidChair specially designed for rough terrain. It cost $300 to build. It changed everything.
