What is Community Support?
The philosophy of our Community Support program is to assist individuals with mental health issues to:
- Participate in supportive and educational activities to increase their satisfaction and success within their community.
- Participate in rehabilitation activities so they can achieve a higher level of functioning.
Levels of Support:
- "High" is 5 or more contacts per month;
- "Low" is 2-4 monthly contacts;
- These services are done in group, one-on-one settings, as well as telephone contacts.
Who will you see?
All staff members involved in the Community Support program:
- Have a minimum of a Bachelor's degree in a Human Service related field;
- Are experienced in working with persons with a chronic mental illness;
- Are required to complete at least ten hours of training which includes information on mental health issues, service delivery and the overall psych rehab philosophy;
- Are supervised and managed by a person who serves as a Qualified Mental Health Professional with a Masters degree in a Human Service related field.
Are you eligible?
You may be if you answer yes to the following:
- Adult ages 18-65;
- Have a psychiatric diagnosis (depression, bipolar, schizophrenia, personality disorder, etc.);
- Deemed appropriate for CSP services via a Psychiatric Evaluation and two simple assessments you and staff do together;
- Live in the community in your own home or with supported living services.
Where are Services Located?
Presently services are provided in:
- Rock Valley;
- Orange City;
- Sioux Center;
- Hawarden;
- Rock Rapids;
- Other communities as arranged.
