Residential Setting  Residential Services

Hope Haven provides a variety of residential services and supports to enable people to participate in the community with the highest possible degree of personal autonomy. The following is a list of services offered. Services have various eligibility requirements and funding options.

ICF/MR (Intermediate Care Facility for the Mentally Retarded)
Westview Homes each serve six people. They have 24 hour staff and the staff will accompany the people receiving this service to the worksite and provide them job support. The goal at Westview is to challenge each person to reach their fullest potential and become as independent as possible in all areas of life. Services are directed toward skill development, including communication, self care, socialization, home maintenance and vocational.
 
Adult Group Homes
Redwood Court in Rock Valley, IA and Northside Court in Hull, IA group homes for adults. Each person has an individual service plan that identifies the support they need and the goals they want to achieve. Staff provides support and instruction. Everyone in these homes participates in employment or another day service between 9 AM and 3 PM on weekdays.
 
Waiver Homes
Homes located in various community neighborhoods across NW Iowa, and usually have 3-4 people living in a house. The people served determine how they want to run their homes (ie: who cooks, cleans, spend leisure time) and each person creates a individual service plan. Staff support them in making these decisions and give needed instruction to help them achieve their set goals.
 
Residential Based Supported Community Living (RB-SCL) for Children
For children ages 8 – 21 years old who have Mental Retardation as the primary diagnosis along with a variety of behavior disorders and mental illness. The setting is a home-like environment where they learn a variety of social skills and independent living skills. They attend school in the local school district and though they aren’t living with their parents, they are a major part of the treatment team. Goal is to help the student become a productive adult able to display acceptable behaviors in society. 
 
Supported Community Living/Family Home
Provided to children and adults who live with their families. The person using this service has identified living goals and the staff provide support and instruction needed to help the person achieve their goals.
 
Supported Community Living/Individual Home
Provided to individuals who live in private apartments and homes. Staff provide support and skills training to each person based on individual service plan.  Typically the training is related to home maintenance, money management, social interaction, self-help skills, medication administration, community integration, etc.
 
Minnesota
Hope Haven provides Supervised Living Setting homes in southwest MN. Each must have a service slot for the MR/RC waiver. Services include training and support in areas of self-help, social, medical, emotional, etc. Staff provide support based on each person’s individual plan and goals.
 
Respite Services
Designed to give the primary caretaker a break from daily responsibilities. Care can be provided in the home of the person serverd or in the home of the respite provider. Hope Haven provides respite services in 11 counties.
